The CSST has determined the company that built a retaining wall which collapsed last year is responsible for the death of a man in his fifties.
Sylvain Mailly was working at 6568 Marquette St. with his boss the day after the retaining wall was built. He was working at the bottom of the job site when the wall, built the day before, suddenly collapsed.
Mailly was crushed underneath dozens of cinder blocks and died at the scene.
The CSST said Michel Jalbert Masonry had built the nine-metre-high retaining wall on Oct. 30, 2013, but failed to put anything in place to ensure the wall would stay in place.
As a result the following day, under moderate winds, the wall blew down.
The CSST has fined Michel Jalbert Masonry for the death, and ordered the company not to build similar structures in the future until it gets proper instructions from an engineer.
The CSST fines companies $15,698 to $62,790 for a first offense.

GEORGE TOWN - Strong west-southwesterly winds whipped up huge tidal waves here, driving the swells into retaining walls at the Esplanade and Gurney Drive.
The giant waves rose higher than lampposts along the promenade, drenching the roads with sea water.
Pedestrians were seen recording and snapping photographs as vehicles hit by the white foamy breakers inched along.
Penang Municipal Council (MPPP) workers and bulldozers were deployed to both locations to clear sand that was washed onto the roads.
At the Esplanade seafront, MPPP worker S. Prakash, 27, slipped and fell when a large wave hit him as he was clearing the pathway filled with sand.
"Throughout my five years with the MPPP, I've never seen such huge waves.
"I panicked when I saw how high they were," said Prakash who did not suffer any injuries.
Over in Batu Ferringhi, the tidal waves wreaked havoc at the food court in Miami Beach, destroying the tiled floor, tables and chairs.
No one was injured in the incident as most of the stalls were closed yesterday for Hari Raya Haji.
A spokesman from the Meteorological Department said the strong winds over Thailand had caused the huge waves to hit the northern states of Malaysia.

Alcohol, mixing decks and a flat screen television were stolen after thieves ransacked a Cromer nightclub.

The break-in at Bouncers Bar on Holt Road, next to the railway station, happened between 3.30am and 2.35pm on Sunday October 5.
Burglars entered the club by forcing open the fire doors at the back of the building.
They stole about 200 worth of spirits, a 32ins Pioneer television and mixing decks.
Investigating officer PC Glen Taylor said: Once inside offenders have trashed the club, smashing glass all over the floor and damaging one of the flat screen TVs while stealing another.
Alcohol has also been thrown over the floor and the glass doors to the fridges have been smashed.
The club was closed at the time.
