The relationship between grief and depression following bereavement has generated much debate in the psychological community and public media. A new article by Rochelle Perper, Ph.D., Grief, Depression and the DSM-5, sheds new light on the subject, and can help both clinicians and the general public achieve a greater understanding of the clinical challenges and options for treating these conditions.

The experience of grief following the loss of a loved one shares several symptoms of major depression, says Perper. However, there are some fundamental differences between depression in the context of grief and the signs of clinical depression. The paper is intended to help clinicians differentiate between a typical grief reaction and the clinical indicators of a major depressive episode, and assist them in identifying signs of persistent complex bereavement disorder in their future work with clients.

The paper also addresses important changes regarding bereavement and major depression to the American Psychiatric Associations DSM-5 manual, which serves as a universal authority for the diagnosis and treatment of psychiatric disorders. Published in May, the DSM-5 now eliminates the bereavement exclusion as part of the diagnostic criteria for a major depressive episode. This leaves the diagnosis to the clinicians discretion when differentiating between a major depressive episode and a typical grief reaction following bereavement.

According to Perper, proponents of this change believe it will aid in diagnostic consistency and provide rapid treatment options to bereaved people who experience symptoms of major depression. Opponents argue that the literature on bereavement is limited, and that bereavement should be distinguished as a unique phenomenon until it is better understood.

The change to the DSM-5 essentially concludes that while bereavement and major depression are separate and distinct conditions, people can grieve and be depressed at the same time, adds Perper. Therefore, clinicians need to carefully consider the differences between normal sadness and the clinical indicators of a major depressive episode following a loss. Furthermore, clinicians who work with bereaved individuals need to become more skilled at making such distinctions in order to recommend appropriate treatment considerations.

The clock is ticking for Harvey Blight to remove two large sheds in front of his home near Menlo Park that officials say give the property a blighted look.

San Mateo County Deputy County Counsel Eugene Whitlock filed an abatement warrant Friday ordering Blight to get rid of the sheds he never received permission to build, in 30 days.

If he doesn't, the county would find someone to do the work and charge Blight for it, Whitlock said Monday, estimating that the cost could run up to $10,000.

Blight couldn't be reached for comment, but he left a voicemail message Tuesday night calling the county's move a "misuse of judicial power."

He also went on to say: "The people without money who suffer the most are people who don't cater to the situation or political circumstances that exist today. When you challenge them and challenge their employment because of their conduct, they'll do anything in their power to retaliate. Not different from Nazi Germany, as far as I'm concerned."

Workers recently were sent to inspect the ramshackle structures but didn't get a close look, Whitlock said. "Harvey kind of chased them off."

According to the abatement warrant, the county first approached Blight in 2005 following complaints that his front yard contained overgrown vegetation, inoperable vehicles and large sheds.

With Blight "kicking and screaming," county officials prodded him to cut the vegetation and remove the vehicles, which included a dilapidated ice cream truck. But Blight drew the line with the two sheds, which Whitlock said he presumed were filled with junk.

The county had enlisted the help of nonprofit Rebuilding Together Peninsula to help Blight clean up and repair the inside of his home.

"Over time, we've been working with Harvey to get those things cleaned up," Whitlock said. "The sheds are still an issue. He wasn't responding to the letters. He was yelling at people."

To the untrained eye, they are green sheds - an incongruous sight on the busy streets of urban London.

But these wooden structures have, in fact, been part of the street-scene in the capital since 1875 - a refuge for taxi drivers to eat, drink and take shelter from the weather.

These days, just 13 remain - and many now serve the general public at large. But cabbies remain their core customer base. Now, as in the 19th Century, they are looked after by the Cabmen's Shelter Fund - a charity.

A grant from the Heritage Lottery Fund will now ensure their place in history is recorded. The Creative Intelligence Agency, a non-profit design and arts organisation, is working with the London Transport Museum to document the shelters' past, gather oral histories from cabbies and commission art works.
