As hospitals across the country prepare for an expected influx of coronavirus patients, officials at Providence Sacred Heart Medical Center (PSHMC) and University of Nebraska Medical Center (UNMC)two hospitals that treated some of the nation's first coronavirus patientsshare their lessons learned, Patti Neighmond reports for NPR's "Shots."
Our analysis: The 'recurring themes' of disease outbreaks
Reports of the newcoronavirusfirst surfaced in early December 2019 in Wuhan, China. According to theWorld Health Organization(WHO), the main symptoms of the virus are fever and lesions in both lungs. Some patients also have reported difficulty breathing, WHO said.
In the United States, state and federal officials as of Wednesdayreporteda total of 129 confirmed or presumed positive cases of COVID-19 and nine deaths linked to the virus, theTimesreports. CDC as of Wednesdaysaid49 of the cases involved Americans who contracted the virus elsewhere and then repatriated to the United States, 24 cases involved patients who had contracted the virus while traveling abroad and were diagnosed after returning to the United States, and 16 cases involved patients who contracted the virus via human-to-human transmission in the United States. CDC said 40 cases were under investigation.
Globally, officialshave reportedmore than 94,300 cases of the virus as of Wednesday, with most of those cases occurring in mainland China, theNew York Timesreports. Officials said as of Wednesday there had been at least 3,210 deaths linked to the virus, and all but 229 of the deaths occurred in mainland China.
PSHMC is treating four patients who were infected with the new coronavirus on the Diamond Princess cruise ship.
Amy Compton-PhillipsEVP and CCO at Providence St. Joseph Health, the hospital network that oversees PSHMCsaid each of the patients are stable, but she noted that "their level of symptomatology is pretty variable."
The patients are being treated in the hospital's special isolation rooms, which are designed to keep the virus from spreading.
"All the air is sanitized before being exhausted out," Compton-Phillips said, "so that germs cannot go from being in the room, through a vent system into the rest of the hospital, or out into the air of the community."
Compton-Phillips said the patients are being treated at the hospital instead of at home as an extra level of precaution.
"They aren't necessarily sick enough to be in the hospital, but because the virus is new and because it's different and nobody knows exactly how it's going to progress and we're still at the phase where we're trying to prevent the genie from getting out of the bottletrying to prevent this from becoming the next big pandemic," she said.
Meanwhile, UNMC has treated 14 patients. According to the hospital, four patients have been cleared and discharged, two patients tested negative for the virus and are still being monitored, and two patients initially tested positive but are now symptom-free and have recently tested negative for the virus.
Like PSHMC, UNMC is treating the patients in special isolation rooms designed to keep the virus from spreading. Shelly Schwedhelm, executive director of emergency management and bio-preparedness at UNMC, said most of the patients have exhibited "minimal symptoms and are stable."
Schwedhelm likened caring for coronavirus patients to caring for severe cases of the flu or other infectious diseases.
"Putting them in a private room, no matter what that looks like is best," Schwedhelm said. "Certainly, getting them masked even before moving them from the front door or reception area is best. While we don't completely know how this virus is transmitted, like most respiratory viruses, the most important routes of transmission certainly are going to be direct contact and droplet."
In a webinar advising hospitals on how to prepare for the new coronavirus, Schwedhelm and a physician colleague told attendees to "do the best that you can and use the resources that you have available in terms of an airborne isolation roombut, if not, do the best that you can, and that is probably going to be good enough in most cases."
Both Compton-Phillips and Schwedhelm noted that resources, including hospital beds and protective gear, could become a problem if the new coronavirus reaches pandemic levels, and cases in the United States surge.
For instance, Compton-Phillips said many hospitals are already strained from treating flu patients. "And so, if we have another (big epidemic of respiratory illness around the same time), it could wreak havoc," she said.
In addition, Schwedhelm said UNMC's 41 isolation beds won't be enough to treat all of the severely ill patients if the case count rises quickly. PSHMC has 10 isolation rooms, Neighmond reports.
"If this virus becomes a virus of pandemic potential, every health system, every organization is going to need to be all in," Schwedhelm said.
Another potential problem for providers is a shortage of protective gear, including gloves, gowns, and masks, Neighmond reports. Compton-Phillips said this is a common problem in a large outbreak because the gear has to be thrown away after it's used.
"The first [coronavirus] patient we hadwhen he was doing well, it took about six changes a day per shift for nurses and caregivers to take on and take off, as they were caring for that person," she said. "And then, as he started getting sicker, it got to 22 changes per shift."
Since much of the protective gear is made in China, "suppliers are triaging to where the need is highest," Compton-Phillips said. She added PSHMC is working hard to be "creative, conservative, and judicious" in its use of protective gear.
That said, Schwedhelm said hospitals and health care organizations across the United States are taking the right steps. They're "doing a lot with education and communication, sharing resources, sharing information," Schwedhelm said, "starting to prepare for that scenario, should it evolve" (Neighmond, "Shots," NPR, 3/2; Lai et al., New York Times, 3/4; CDC website, 3/4).

Dr. Sarah Rhoads MD16 RES20, a University practitioner who is a resident at Hasbro Childrens Hospital, has several tricks up her sleeve for helping children who come into the hospital with asthma exacerbations.
When young patients exacerbations begin to subside, Rhoads asks them to blow bubbles or puff on pin wheels. This helps them practice taking deep breaths, making sure theyre keeping their lungs open. Sometimes shell give patients Popsicles. When a Popsicle melts it can be a secret way of keeping a kid hydrated, if theyre willing to take the bait, she said.
Mnica Huertas, a Providence resident, has three children who have asthma. She and her children have started calling the doctors at Hasbro the Popsicle doctors.
From the moment a child enters the emergency room with an asthma exacerbation, the medical staff at Hasbro launch into a well-rehearsed protocol. It is a situation they see often: Asthma is the most common chronic illness among Rhode Islands children, The Herald previously reported. Asthma also disproportionately burdens urban, low-income communities of color.
For a child with chronic asthma, the illness can have multiple consequences, from Intensive Care Unit stays to school absences. In addition, asthma medication and ER visits can impose a financial burden on families.
Most children who come into the hospital with asthma exacerbations receive an albuterol dose, which helps open up their airways and is found in prescription rescue inhalers. Sometimes children also receive steroids to calm inflammation. Most kids at that point are doing okay, Rhoads said.
The staffs goal is to make sure that children can go four hours or more without needing albuterol. If their condition necessitates breathing treatment every one to two hours, they get transferred into more intensive treatment areas.
In that scenario, Rhoads will communicate the message to the family by telling them that their child has shown that he is getting a little bit better, but we think he needs to be watched closely.
Dr. Nicholas Nassikas MD 15, a University Pulmonary, Critical Care and Sleep Medicine fellow specializing in asthma, treats patients who have been transferred to the ICU for asthma exacerbations. He helps the ICUs staff navigate life support machines that can provide oxygen for those patients, or even breathe for them. Asthma is scary because it can get very bad very quickly, he said.
In 2016, when Rhoads was an intern at the hospital, a girl suffering from an asthma crisis was brought into the emergency room by Emergency Medical Services. She was having such a hard time breathing that she couldnt even talk. The girl was taken into a specialized room within the ER where she received heightened treatment before being transferred to the ICU. The breathing treatments she received helped her recover and she went home a day or two later, Rhoads said. It was the first time in her medical career that she had witnessed a situation where a child with asthma ended up in the ICU.
In most cases, children leave the hospital relatively quickly, Rhoads said. When a child who is having a really tough time breathing receives a few simple therapies that make them feel better, and they end up running out the door, that feels great, she said.
After an asthma exacerbation, the child leaves the hospital with a piece of paper explaining how to treat asthma if it happens again. One of the hardest parts of Rhoads job is seeing children come to the emergency room repeatedly because their asthma is not well controlled.
Sometimes she has to write absence notes for those children if they missed school while they were in the hospital because of asthma.
Asthma contributes to school absenteeism, according to Dr. Daphne Koinis-Mitchell, professor in the departments of Psychiatry and Human Behavior and Pediatrics and director of the Community Asthma Program at Rhode Island Hospital.
We see the rates of absenteeism are much higher in children with asthma in urban school districts compared to children without asthma, she said, adding that children with asthma miss, on average, two more weeks of school than children without asthma. In some districts with high asthma prevalence, this difference is larger.
There are multiple factors causing asthma-related absenteeism, just as there are multiple factors causing asthma itself.
After spending time in the hospital, children might be tired and parents may choose to keep them home. Some caregivers do this for fear that (their children) may experience asthma symptoms when theyre in a school setting, Koinis-Mitchell said.
Even if they go to school, lost sleep due to asthma can affect a childs academic performance, because it can make it harder for them to pay attention and concentrate in class, she said.
In order to help families better control their childrens asthma, Hasbro has established programs on asthma management education tailored to caregivers and children. Some of these efforts have existed for over 20 years, Koinis-Mitchell added.
One of these, the Rhode Island Asthma Integrated Response (RI-AIR), is one of only four programs of its kind in the country and receives federal funding. RI-AIR identifies children with asthma living in high-risk areas, conducts screenings and designs tailored, evidence-based interventions. If (families) adhere to the intervention, their asthma outcomes will improve, Koinis-Mitchell said, adding that these outcomes include school absenteeism and academic performance.
The intervention might include coordinating with the school nurse and other staff at the childs school.
Some schools have multiple systems in place to help address their students asthma. Individual schools have target teams, made up of administrators, social workers, the school nurse and other staff members, who regularly analyze attendance data and suggest interventions for students with high absenteeism, wrote Laura Hart, Providence Public Schools District director of communications, in an email to The Herald. If the cause of a students high absenteeism is asthma management, the school will often reach out with additional referrals and supports.
Beyond affecting their school performance, a childs recurring asthma can also have a high financial cost for their family.
Huertas, the Providence mother whose children frequent the Popsicle doctor for their asthma, has seen these costs firsthand. On her healthcare plan, the co-pay for chewable steroid tablets is $5, and inhalers cost her $35, she said. She buys four inhalers a month.
In addition, Huertas estimates that she pays $100 every time one of her children goes to the emergency room. Shes been there 11 times in the past three years, bringing the cost up to $1,100, she estimated.
This cost can impose a financial strain on some families. A few years ago, an adult family friend of Huertas with chronic asthma died from an exacerbation, leaving two children behind, because she couldnt afford her pump medication, Huertas said. I was like, heres an albuterol, take mine, but I guess it just wasnt enough. She died from a(n) asthma attack, its so sad, and I always get scared, I take my kids right to the doctor.
And at the doctors, the cycle of asthmas effects on childrens everyday lives begins all over again.

A 9,500-square-foot, fully-accessible playground could open in Shipyards Park in 2021.
Whitehorse city council has been asked to accept the gift of the playground from the Canadian Tire Jumpstart Charity.
In a presentation March 2, Jumpstart ambassador Stephanie Dixon told council members about Jumpstarts initiative that started in 2017 to build a fully, accessible and inclusive playground in each province and territory in the country.
Since then Jumpstart Playgrounds have been built in Charlottetown; Winnipeg; Calgary; Toronto; Prince Albert, Sask.; Surrey, B.C.; and Saint John, N.B.
Dixon first visited the Jumpstart playground in Surrey last year.
Everything is double wide, she said as she recalled the space available for those in wheelchairs to maneuver where they want to go.
For those with hearing devices who cant normally use a metal slide due to issues with static electricity theres roller slides that eliminate static.
For those who on the autism spectrum or with sensory processing issues or who may just simply get overwhelmed in larger crowds a dome was in place as a quiet place.
Its a place where every child can participate and ensuring that happens at an early age is important, Dixon said.
On the playground is where we develop our sense of self, she said.
As the city works to make the community more accessible, why not start at the beginning with a playground, Dixon said, adding she hopes council members see the magnitude of what the playground would bring to Whitehorse.
The Whitehorse playground would be located inside the skating loop at Shipyards Park. At 9,500 square feet, it would be close to the same size as the 9,859 square foot playground at Rotary Park.
Jumpstart would design, supply and install the playground, including the rubberized surface, with the city then responsible for the ongoing maintenance. The contract between Jumpstart and the city states the city would be required to keep up maintenance for the parks 15-year life span.
Shipyards Park was selected as it has accessible parking, an asphalt surface, washroom facilities and a water fountain. As such it is the only spot in Whitehorse that meets all the criteria for a Jumpstart playground.
As Landon Kulych, the citys parks and community development manager, said there is also staff on hand at Shipyards Park each day, which means there will be more eyes on the playground. He also said the parks location inside the skating loop means it will not hinder events set to happen at the park.
While council members were clear in their gratitude for the $1 million playground, Coun. Laura Cabott also raised questions about the maintenance costs stating her desire to ensure the city is ready to take it on.
City manager Linda Rapp said the costs would be absorbed into the overall operating budget for parks while Jumpstart associate vice president Marco Di Buono said most of the equipment installed into the playground come with a 15-to-20-year warranty. The equipment is also not overly engineered, he said, making it fairly simple to fix when there are issues.
Whitehorse city council will vote March 9 on whether to sign off on the agreement for the playground.
If the agreement is signed, construction of the new playground would begin this year with the finishing touches being put in place in the spring of 2021. It would then be open to the public.

Opening to the public next week, Edge is the highest outdoor observation deck in the Western Hemisphere.
The sky deck juts out an exhilarating 80 feet from a glass-facade building in Hudson Yards on the far West Side of Manhattan in New York City. Over a thousand feet in the air, the venue opens on March 11, but Yahoo Finance got a sneak peak during Rolling Stones private Women Shaping the Future event on Wednesday.
Edge is designed to take visitors out of their comfort zone to experience New York like never before, said a joint press briefing by its developers Related Companies, Oxford Properties Group, NYC & Company, KPF and Rockwell Group.
Bolted between the east and south sides of 30 Hudson Yards, the 7,500-square-foot deck sits 1,131 feet in the air. A video shows sketches of Hudson Yards on graph paper, coming to life, as guests jet up an ear-popping 60-second elevator ride to the 100th and 101st floors.
The Edge juts out 80 feet from the building in two tiers the entrance is a steep staircase up from the main observation deck made of triangular panels. The windy yet hefty sky deck is 765,000 pounds, made of 15 sections, each weighing between 35,000 and 100,000 pounds. Rather than competing with the skyline views, Edges sleek, minimal design puts New York City in the spotlight.

The harrowing view below is hard to miss, with 360-degree views through angled windows that allow guests to lean out overlooking the city. The nine-foot-tall glass walls are made of 79 panels, totaling 3,000 square feet of glass. Each panel weighs 1,200 pounds and is angled 6.6 degrees outwards.
For the most daring guests, a 225-square-foot triangular floor has a glass panel that offers a view straight down 100 floors to taxicabs and construction cranes below.
Edge is the only sky deck that hangs off a building rather than being on the roof, said Horkin. The result is an experience where you are completely surrounded by sky... [You can] stand on the glass floor and look straight downits breathtaking no matter how many times you do it.
Just above the sky deck is Peak, a restaurant and bar by London-based rhubarb hospitality group that offers floor-to-ceiling views of New York City from the 101st floor. The restaurant offers small, intimate tables and a marble countertop bar appointed by a silver, crescent sculpture. Modern, angular lighting fixtures and silver-and-gold accents pair with cozy carpeting and velvet seating.

The restaurant wraps around one side of the building, and the other side functions as an event venue, with marble floors and circular, gold hoop lighting. The venue also has a small, sleek bar downstairs, on the 100th floor.
The Edge will open to the public on March 11, and standard adult tickets will cost $36. The building and space is part of the Related Companies Hudson Yards development, a massive multi-use neighborhood with over 100 shops and restaurants. The Oxford Properties Group, NYC & Company, and KPF and Rockwell Group partnered with Related Development in the $25 billion development project.

Aggro decks are great to quickly climb the ladder as the games are generally faster. Snapconcede/modo here has chosen the trusty Rakdos Knights to achieve the task, despite the deck falling out of favor for Mono Red Aggro. The deck has better and bigger creatures overall, and Rotting Regisaur + Embercleave is still a deadly combo.

After the 2016 presidential election, political airwaves seemed dominated by two refrains:
Donald Trump is our president!
Actually, Hillary won the popular vote.
Both statements are equally valid. Despite their gripes, liberals could not deny that Donald Trump would occupy the Oval Office. On the other hand, perhaps Republians should have qualified their enthusiasm by acknowledging that their candidate spoke for a minority of the American people.
These opposing outlooks are rooted in an underlying issue that is more complex than mere partisanship. The 2016 election is proof of a system that disproportionately rewards the rural vote. In vying for control of the House of Representatives, the Senate and the presidency, urban political voices are at a disadvantage because of the outsized influence that sparsely populated states hold in our electoral system. The result is that representation in the federal government fails to adequately reflect public opinion. In an age of polarization along geographical lines, Democrats are the ones who lose out. If they hope to win in 2020, Democrats need to take into account this deep-seated rural advantage and shift their strategy to win more rural votes.
The numbers demonstrate this disconnect between the electoral and popular votes. Over the past few elections for the House of Representatives, Republicans have won a share of seats that is four to five percentage points higher than the portion of the popular vote they garnered. In the 2018 midterms, even as many Republican seats were taken by Democrats, their losses were hedged by gerrymandering according to one analysis, Democrats current advantage in the House would have been 16 seats larger if congressional boundaries had been drawn fairly.
The Senate, where Republicans control 53 seats, is even less representative of the popular vote: The 53 GOP Senators in the hundred-member body represent about 15 million fewer people than the 45 Democrats who sit across the aisle. And in the Electoral College, the five most rural states in America have twice as many electoral votes per resident as the five largest states do.
Believe it or not, the disproportionate representation of small states was a deliberate part of the Framers original plan. The Senate, for instance, was intended to guarantee that small states have a meaningful influence in government and are protected from the tyranny of the majority. The Electoral College was designed as a concession to Southern states, protecting the political influence of their voting populations while leaving slaves disenfranchised. The design of the Electoral College system was not a real problem until the 2000 presidential election, when George Bush won the White House through the Electoral College despite earning fewer votes than Al Gore. This was the first time since 1888 that the popular vote and the Electoral College produced opposite verdicts. But now that the previous two Republican presidents won in this manner, it is time for Democrats to take notice.
Why now, at the start of the new millennium, does the Electoral College fail to represent the views of the majority of Americans? Over the past few decades, geography has increasingly come to predict party affiliation: The Democratic Party now largely represents cities, while the Republican Party largely represents rural America. Before this sociopolitical sorting intensified at the end of the 20th century, the Constitutions bias against voters in major population centers affected both parties about equally. Republicans depended on moderate Eastern elites, such as the Rockefeller Republicans, as much as they did on social conservatives to form winning coalitions. Democrats, on the other hand, could count on urban political machines such as Tammany Hall, even while they also relied on the Solid South for victories, until they lost Southern support with the passage of the Civil Rights Act.
But the era of parties that crossed geographic lines is now a distant memory. The 13 most densely populated states are represented by 121 Democrats and 73 Republicans in the House. The other 37 less densely populated states are represented by 163 Republicans and 72 Democrats. The Republicans are definitively the party of rural America, and so they benefit from the advantages in representation that are built into the Constitution. The Democrats, standard-bearers of cityfolk, must deal with the ramifications.
The upshot is that while the Democrats should be cognizant of the systemic disadvantages they face, they need to reshape their political strategy to better suit the needs of moderate and even conservative voters in places such as the Rust Belt to compete outside of states like New York and California. In practice, they will have a hard time doing so because of the growing progressive wing of the party, which could alienate rural voters who might otherwise consider voting blue.
The challenge facing the Democrats thus adds a new twist to an important question: What exactly do working-class Americans care about? Even if the structural reforms presented by candidates such as Bernie Sanders are popular in urban centers, can they win over enough moderates to overcome our governments bias against the political influence of the coasts? Just as important: Is Bidens time-tested message to the Rust Belt enough to rebuild the blue wall?
The two wings of the Democratic Party offer different answers, and it is not clear which will more effectively resonate with voters. But if there is one thing that our broken system of uneven representation should teach Democrats, it is that they need to tailor their message to the highly diverse geographies around the country, not just to the urban centers. Even outside of the cities, our country moves. It breathes. It votes.

Joo FrancofromBelow Deck Mediterraneanshared that he was ready to live a sober life. He revealed that he quit drinking alcohol after doing a considerable amount of reflecting, he shared on Instagram.
He started a series of three posts over the weekend. His first post focused on how he spent too much time dealing with a hangover. He could have instead been enjoying life with his family.
Ill tell you what A hangover, he remarked along with photos taken with his father. One thats lasted 3 days! Demons Alcohol. Ive decided I hate this feeling! I seriously hate it! I should be the happiest I have ever been and yet I feel insecure, questioning my purpose, or more so, what Im doing with my time that is important and how much of my time on this earth I am spending on unimportant things.
His next post dove deeper into why he needed to stop, citing that signs were everywhere.
Franco began his post revealing that there were times hed wake up covered in blood after a night of drinking. Or getting arrested should have been the wakeup call he needed. We are given signs that we choose to see or ignore. I have certainly been given my fair share of signs, he wrote. They come to me in different ways. Some are a blatant smack in my face, like getting in trouble with the law or waking up with torn clothes, blood and gashes all over my body, some blood stains not my own. Broken knuckles and no idea of how it happened.
Seeing Jezebob emerge on Below Deck Med should have also been a sign, he added. Or watching my actions on a TV show, knowing very well that I would have handled situations very differently if I was sober, he penned.
But also learning that Captain Sandy Yawn was sober should have been a sign too. Some signs more subtle, maybe through other people like@captainsandrayawngiving me light on when and why she made her decision to quit alcohol. The people close to me, telling me that I have so much potential to be great, I just need not f**k it up by going to jail or having a car accident.
Franco says his demons were far more serious. Not just killing me but all those involved too, he admitted. Or worse- I live and others die. I go through waves. I reflect and calm down for a few months and then something gets me on the road to inevitable doom again.
Like some people, he shared that he often was able to get off the hook I SOMEHOW miss a date with the devil and come back to my senses, all to start the process again. I have been told a countless amount of times that I have an endless supply of lives because SOMEBODY just keeps the lives coming! But why??
Franco said he cheated death too many times. I should have died this weekend with my actions, he revealed. I should have died a month ago on my way to Victoria falls when I drove with my best mates in the car. He added, I can count at least 10 times a year that I should have died.
Yawn saw Francos post and told him she was here for him. Joao, after reading this I realized that this has always been your struggle, she responded. I promise you, from experience, if you stay clean and do the next right thing, your life will be beyond your wildest dreams! I love you and Im here for you. Yawn mentored another crew member last season who would get blackout drunk when the crew went out on the town.
Last season she shared that she was a recovering addict. Yawn told Showbiz Cheat Sheet she too could have had a different life if she continued to drink. I was a mess, she said in August. By the grace of God, Im alive. No matter how much money you have, its not going to save your life if you are an addict or an alcoholic. And I was a major addict and alcoholic.
Like Franco, she never hurt or killed anyone. But I could have killed someone. My life would be very different right now. Deckhand and close friend Colin Macy-OToole cheered Franco on too. Proud of you buddy! You got this! , he wrote.
