Most Australians face the prospect of paying more to visit their doctor, despite the federal government ditching its plan for a $7 GP co-payment.
Instead, it will be up to doctors to decide whether to impose an optional $5 co-payment on their bulk-billed patients to make up for a reduction in the rebate they receive from Medicare.
Patients who are not bulk-billed will have their rebate reduced by $5 as well.
Pensioners and other concession card holders, children under 16 and those living in aged-care facilities will be exempt from the reductions.
Labor slammed the modified plan as a back-door way of introducing a co-payment, while the Greens accused the government of forcing doctors to do its "dirty work".
Prime Minister Tony Abbott, who announced the change on Tuesday, said it was "pretty obvious" the government's initial plan needed reworking.
"I am absolutely convinced that the package that we have brought forward today is a better package," he told reporters in Canberra.
He also announced the government was addressing what he labelled "six-minute sausage machine" medicine - a move welcomed by the Public Health Association of Australia.
Consultations of less than 10 minutes will attract a Medicare rebate of $11.95 only, and a rebate of $32.05 will apply for standard consultations between 10 and 20 minutes.
The rebate changes do not need parliamentary approval, but they could be disallowed by the Senate if Labor and the Greens get sufficient support from the crossbench.

In many cases the thefts are captured on tape. Homeowners can see brazen thieves swiping the gifts and boxes from their front porch just after they are delivered to their homes.
It happened to Denee Forbes. She was inside her house in Rochester and never saw what happened until she checked her surveillance video.
Sure enough, the cameras outside the Forbes home caught the entire crime on tape. That video helped police catch the suspect, who was charged with similar crimes in the City of Rochester and Irondequoit.
Since that happened back in September, the Forbes are more careful when leaving packages on their porch. Denee knows when her items will be picked up and delivered, so nothing is left for long periods outside.
Forbes also leaves her drapes open so she can see what's happening in front of her house. She advises other homeowners to get security cameras to protect their property.
Not only are thieves stealing property, in some cases they are following delivery trucks and then waiting until they leave, to snatch the items.
Companies like FedEx said customers can take steps to protect their deliveries. You can customize the delivery by pinning down when it will arrive or asking that someone sign for the package before it is left on the porch. You can also ask a neighbor to pick up your deliveries or ask the delivery company to leave it in a less obvious spot.
Forbes said the sad thing is that for her door-to-door delivery is a must. She runs a business from her home. It's convenient. She doesn't want to have to pick up items from Fed Ex or UPS.

A new Angies List survey of more than 1,000 service providers shows home improvement contractors are optimistic about business in the coming year.
We asked contractors what their conversations with customers were showing, and 62 percent of them say their customers arent just wanting to make improvements next year, theyre willing to spend more on them than customers spent in 2014, said Angies List Founder Angie Hicks.
More than half of the contractors said customers were planning to add $5,000 to what the average project cost last year, she added.
The online survey, conducted in November among highly rated companies specializing in home improvement, asked about remodeling trends for 2015, as well as whether homeowners budget expectations had changed.
The breakdown of how much customers are planning to spend on remodeling projects next year compared to what customers spent this year:
When it comes to the projects themselves, most are being planned involve kitchens, bathrooms, bedrooms and lighting. Customers are asking for bigger closets and pantries. When it comes to bathrooms, fiberglass around the tub and shower is out and tile is in.
Wed been hearing occasionally that homeowners are getting tired of cleaning the fingerprints and smears off stainless steel appliances, Hicks said. The survey, however, showed 70 percent of contractors think the shiny stuff is still in and several of them mentioned fingerprints are worse in lower quality products.
Contractors told Angies List the average bathroom remodel now means customized luxury, and homeowners are willing to pay more for it.
One contractor said his usual bathroom remodel project was in the $15,000 range for years. His average bathroom project is now starting at $20,000. Another contractor said its not unusual for homeowners to spend $50,000 on high-end facilities.

As a church you have demonstrated that you are committed towards the development of your church. I am told that you managed to contribute over K14 million out of the budgeted K30 million for the building of the church, this is very commendable, said Chilima.
He said God only gives to those who work hard as such he advised the churchs faithful to continue working hard and praying if they have to achieve their vision of having a magnificent church.

Chilima then took advantage to update the nation on the current economic status of the country, saying government has no money and that Malawian should not be scared of the situation as most countries including donor countries in the world are also facing economic hardships.
There is nothing you can achieve by pin pointing the government for the economic challenges we are going through, these problems have not left out any countries including donor countries, he said. On donors, the Vice President said donors attach conditions to their assistance and that such conditions are just scape goats, adding that the donors themselves are also in financial hardships.
This is not to scare you, but this is the truth and what we should do as Malawians, is to work hard so that our country becomes economically independent, he advised.

With the holidays here, people often feel the urge to donate time or money to the less fortunate. However, there are organizations that center their time around giving - well beyond the holidays.
One of those is Heritage Baptist Church in Hiltons, Va.
"The needs exist all during the year," Pastor Larry Browder said. "Sure, the holidays themselves are special and there is a need there, but the need of the people exist year-round and not just during this time of year. This is an ongoing thing and it is daily and weekly, people have physical needs and you never know when it is going to happen but it does happen all year long."
Browder has been pastor at Heritage Baptist Church since 2003, after being a member of the church since 1983, and says his church takes it upon themselves as a mission to serve.
"We feel like our central guide comes from what Jesus wanted us to do," Browder said. "We feel like it is a mandate from his teachings and that is what we strive to do."
The group takes on multiple tasks, including Hiltons Helping Hands, a benevolent ministry funded by the local churches, helping needy families. Another is Blanket America Ministries, a Charlotte, N.C. based ministry that gives away new blankets, coats, Bibles and socks annually with a strong emphasis on evangelism.
"We were contacted by each one of those and asked to be a host church," Browder said. "And we wholeheartedly endorsed these because it goes along with the mandate of what Jesus wanted us to do. Because these were things that were badly needed in the community."
They also do monthly visits to local nursing homes where they share music and devotional.
