"One simple, dynamo-technology, low voltage switch stood between the United States and a major catastrophe."
That is the blunt 1969 assessment of Parker F. Jones, the then supervisor of the nuclear weapons safety department at Sandia National Laboratories, in a newly declassified document that sheds light on a 1961 accident in which the United States almost nuked North Carolina.
The document was given to The Guardian today by author Eric Schlosser, who came upon the document doing research for a new book.
The story of Goldsboro, N.C. is well known. It was the Cold War and the U.S. had B-52 bombers in the air as a precaution, ready to strike if the Soviet Union made a move. As Rudolph Herzog explained it in his book A Short History of Nuclear Folly, one of the B-52s was carrying two Mark 39 hydrogen bombs, which are many times more powerful than the bomb dropped on Hiroshima. Like the others, the B-52 flew huge circles around the U.S. and the Atlantic, needing to be refueled along the way.
The plane had reported an oil leak in its left motor.
"The third time [it was filled up], a filler neck broke off the plane ... and kerosene began pouring from its right wing," Herzog writes.
To make a long story short, the plane disintegrated and the two bombs fell from the sky onto American soil.
As The Guardian reports in its piece about the document that the "U.S. government has repeatedly publicly denied that its nuclear arsenal has ever put Americans' lives in jeopardy through safety flaws."

We're four months out from the Moore tornado but volunteers are still busy trying to help victims in their path to recovery.
Countless tornado victims lost so much in the May storms, mainly space, and that is why one group is working to give them a place to store things.
The foundation is laid on the new home Joshawa Green is rebuilding in Moore, four months after the May 20 tornado. Green needed a shed too for storage, and the group Sheds for Hope' answered his call for free.
"They had it built right before I even knew they were going to and never got a chance to thank them really," Green said.
In a warehouse, Sheds of Hope volunteers are making way for a special delivery. They're preparing to build 15 more sheds for storm victims.
"So they have a place to lock their belongings to prevent looting and give them a place to start rebuilding on their property," Laura Atherton, coordinator for Sheds of Hope.
Atherton heads the group, which is a part ofthe Presbyterian Church in America. She'll be in charge ofthe100 college students volunteering to put together the sheds piece by piece on Saturday morning.
"They will assemble the shed, put shingles on it, paint it the color that the homeowner has requested," Atherton said. "Then later the homeowners will drive by later that evening and have a brand new shed on their property ready to go."
It's a tedious project that will take about four hours to complete. So the group is getting all their tools and materials stacked in a row, ready to make some Moore resident's day. Like Green, who says he's grateful for the secure storage space.

Cluster flies get their name because they enter a home or building in the fall and gather together in clusters. While they look like common house flies, theyre actually larger and slower.
According to Horacio, most people dont even realize they have cluster flies until they become active in early spring or on warm and sunny wintry days. Thats because cluster flies are attracted to light when the temperature is above 10C and to dark areas and other flies when the temperature is below 10C. For families that head up to their cottages in the winter, cluster flies can emerge en masse when the heat is turned on.
Just because cluster flies are, for the most part, dormant over the cold autumn and winter months doesnt mean its a good idea to ignore them, Horacio adds. Homeowners should also be careful if they want to take care of the issue themselves. They may think using off-the-shelf insect or bug bombs to eliminate cluster flies and other pests is a good idea, but this can backfire. A large number of dead cluster flies inside the walls and other hard-to-reach places can provide a steady diet for other pests, including mice and carpet beetles.
Overwintering pests like cluster flies, Horacio explains, will try to enter a structure through cracks and gaps around windows or doors, or where siding connects to the foundation or root. Common entry points also include attics and chimneys. To help reduce cluster flies, its important to hire an experienced pest control company to treat not just the interior of a home, but also the exterior.
By taking proactive measures, property owners can reduce the presence of cluster flies and other pests in search of a warm hiding place. Protecting a home from overwintering pests like cluster flies can also decrease the chances of an influx of secondary pests like mice, Horacio concludes.
